Indications/Uses
Prevention of postpartum haemorrhage due to uterine atony.
Dosage/Direction for Use
IM/IV Vag delivery 100 mcg/mL as single dose IM or by slow IV inj over 1 min. Administer only after delivery of the infant & as soon as possible after delivery, preferably before delivery of placenta. IV Caesarean section under epidural or spinal anaesth 100 mcg/mL as single dose by slow IV inj over 1 min. Administer only after delivery of the infant & as soon as possible after delivery, preferably before delivery of placenta.
Contraindications
Hypersensitivity to carbetocin or oxytocin. Serious CV disorders. Labour before delivery of the infant. Induction of labour. Pregnancy.
Special Precautions
Not appropriate to use at any stage before delivery of the infant. Consider additional therapy w/ another uterotonic in case of persisting uterine hypotonia or atonia & consequent excessive bleeding. Epilepsy, migraine, asthma, CV disease or any state in which rapid addition to extracellular water may produce hazard for an already overburdened system. Carefully monitor patients w/ eclampsia & pre-eclampsia. Possible hyponatraemia particularly in patients receiving large vol of IV fluids. Recognize early signs of drowsiness, listlessness & headache to prevent convulsions & coma. Gestational DM. Adolescent & childn <12 yr.
Adverse Reactions
Headache, tremor; hypotension, flushing; nausea, abdominal pain; pruritus; feeling of warmth. Anaemia; dizziness; chest pain, dyspnoea; metallic taste, vomiting; back pain; chills, pain.
Drug Interactions
Risk of severe HTN when given 3-4 hr following prophylactic administration of a vasoconstrictor in conjunction w/ caudal-block anaesth. May enhance BP-enhancing effects of ergot-alkaloids (eg, methylergometrine). Hypotensive effect may be enhanced & effect on the uterus may be weakened by some inhalation anaesth (eg, halothane, cyclopropane). Possible arrhythmias for oxytocin during concomitant use.
